If you’ve been dealing with a chronic health condition, you may be eligible to receive Medicare rebates for physiotherapy through a GP Chronic Condition Management Plan (GPCCMP).

Management Plan (GPCCMP) Help You?

A GPCCMP is a formal care plan prepared by your GP for anyone with a medical condition that has lasted (or is expected to last) 6 months or more. It outlines treatment goals and refers you to allied health professionals—like us—for support. We use it to help people reduce pain, improve range of motion and confidence with their body.

Am I Eligible?

A GPCCMP is a formal care plan prepared by your GP for anyone with a medical condition that has lasted (or is expected to last) 6 months or more. It outlines treatment goals and refers you to allied health professionals—like us—for support.

You may be eligible if:

  • You have a chronic or terminal medical condition

  • Your GP has created a current GPCCMP

  • Your GP has referred you for physiotherapy

Your GP will assess your condition and decide if physiotherapy is part of your long-term care needs. These plans are reviewed annually.

How Many Sessions Are Covered?

Medicare rebates can be claimed for:

  • Up to 5 allied health sessions per calendar year

  • All 5 sessions can be used for physiotherapy if approved by your GP

  • Additional services like podiatry or dietetics may share those 5 sessions

  • Aboriginal and Torres Strait Islander patients may be eligible for up to 10 sessions

Unused sessions do not carry over into the next year.

What’s the Rebate Amount?

As of July 2025, the current rebate for physiotherapy is:

  • $61.80 per session (Medicare Item 10960)

Please note:

  • Our standard fees may be higher than the rebate

  • Any difference is your gap payment

  • You can pay at the time of service, and we’ll process the Medicare rebate for you on the spot via Tyro or HICAPS (rebate goes into your debit account)

How Long Does a Referral Last?

  • Referrals created under the new GPCCMP system (from 1 July 2025) are valid for 18 months

  • You can use the sessions across multiple providers if needed

  • If you have a plan from before July 2025, it remains valid until 30 June 2027

How to Claim Your Physio Rebates

  • Visit your GP – Ask for a GP Chronic Condition Management Plan with a referral for physiotherapy

  • Book with PhysioCall – Call or book online and let us know you have a Medicare referral

  • Attend your appointment – Bring your referral and Medicare card

  • Pay & claim – You’ll pay our normal fee, and we’ll process your Medicare rebate immediately

What conditions are covered?

The GPCCMP covers any condition that’s lasted 6 months or more — including arthritis, chronic pain, sciatica, postural issues, and tendon injuries.

Can I combine this with a private health fund?

No — you can’t claim both Medicare and private health on the same session. But you can alternate or use private health once Medicare sessions are used up.
