What Is a GP Chronic Condition Management Plan (GPCCMP)?
If you’ve been dealing with a chronic health condition, you may be eligible to receive Medicare rebates for physiotherapy through a GP Chronic Condition Management Plan (GPCCMP).

How Can GP Chronic Condition
Management Plan (GPCCMP) Help You?
A GP Chronic Condition Management Plan sets out care goals and recommended treatment for an eligible chronic or terminal condition. Your GP decides whether a plan and referral are appropriate. Physiotherapy may form part of that care when it is consistent with your plan.
Am I Eligible?
Eligibility depends on your condition, care plan and referral. A diagnosis alone does not guarantee Medicare-funded physiotherapy.
Eligible older GP Management Plans and Team Care Arrangements prepared before 1 July 2025 can continue until 30 June 2027. See our Medicare physiotherapy guide for the transition and referral requirements.
You may be eligible if:
You have a chronic or terminal medical condition
Your GPCCMP has been prepared or reviewed within the previous 18 months
Your GP has referred you for physiotherapy
Your GP will assess your condition and decide whether physiotherapy is appropriate. The plan can be reviewed or updated as your needs change.
How Many Sessions Are Covered?
The standard limit is five individual allied health services per calendar year, shared across eligible professions.
Eligible Aboriginal and Torres Strait Islander patients may access up to ten services in total through the relevant pathways, including at most five under the standard chronic-condition items. Check your referral and remaining entitlement before booking.
New referrals do not have to allocate a fixed number of services. Your GP can specify a number, and you choose how to use your Medicare benefit within the referral and eligibility requirements.
Unused sessions do not carry over into the next year.
What’s the Rebate Amount?
From 1 July 2026, the current Medicare benefit for physiotherapy is:
$63.40 per eligible session (MBS item 10960, effective 1 July 2026)

How Long Does a Referral Last?
Referrals created from 1 July 2025 are valid for the timeframe stated by your GP, or for 18 months from the first service when no timeframe is stated.

What is a GPCCMP?
A GP Chronic Condition Management Plan is prepared by your GP for one or more conditions that have lasted, or are likely to last, at least six months, or are terminal. It may support access to MBS-funded allied health services.
Am I eligible?
Eligibility is a clinical decision made by your GP. Your GP will consider whether your chronic condition would benefit from a structured plan and allied health support such as physiotherapy.

How much does Medicare cover per session?

How many sessions can I claim per year?
Under the chronic condition pathway, eligible patients can usually claim up to five individual allied health services per calendar year, shared across professions. Eligible Aboriginal and Torres Strait Islander patients may access up to ten. Your GP and clinic can check which items and remaining services apply to you.
Can I choose where I go for physio?
Yes. A referral to physiotherapy can be taken to an eligible physiotherapist. You can ask your GP to refer you to PhysioCall Gladstone.
How is the Medicare benefit processed?
Bring your referral and Medicare card. PhysioCall can process the Medicare claim electronically via Tyro or HICAPS after payment, with the benefit paid to your nominated account.
What conditions may be eligible?
The plan is for chronic or terminal conditions. Examples may include arthritis, persistent pain, sciatica or tendon conditions, but your GP determines whether you meet the requirements and what services are appropriate.
Can I combine this with private health insurance?
You cannot claim Medicare and private health insurance for the same session. You may use private health for a different session, subject to your policy and the provider’s billing arrangements.
